Welcome to First Choice VIP Care Plus' (Medicare-Medicaid Plan) Care Management department. A Care Coordinator is available to help explain the benefits and services of your health plan and support you as you try to stay healthy. Your care coordinator will help organize, plan, and connect you to the health services you need. He or she will also check on your treatment and help you transition from hospital to home. He or she can help you get to provider visits and follow providers' orders.

Because your Care Coordinator tracks your care and helps you take charge of your health, the quality of your health care improves. It's another way that First Choice VIP Care Plus is here for you.
