As a participating plan of Healthy Connections Prime, First Choice VIP Care Plus is required to provide our members with meaningful access to medical programs and services. First Choice VIP Care Plus is required to reasonably accommodate enrollees and ensure programs and services are as accessible (including physical and geographic access) to an individual with disabilities as they are to individuals without disabilities.

First Choice VIP Care Plus asks that providers complete this ADA survey for each physical practice location to ensure we are providing First Choice VIP Care Plus members with the most accurate information regarding your practice. You may complete the survey the following ways:
